Carl V. Hill, PhD, MPH, is the chief diversity, equity, and inclusion officer for the Alzheimer’s Association, overseeing strategic initiatives to strengthen the Association’s outreach to all populations and providing communities with resources and support to address the Alzheimer’s crisis.

In this role, Dr. Hill is responsible for driving Association-wide messaging and mobilization efforts addressing systemic health inequities in treatment, clinical trials, and research. With support from the National Institute on Aging (NIA), Dr. Hill led the Association’s work to convene the annual Promoting Diverse Perspectives: Addressing Health Disparities Related to Alzheimer’s and All Dementias conference, which brings together researchers from around the world to examine disparities in dementia prevention, diagnosis, and treatment.

Dr. Hill also develops cross-functional partnerships with organizations to advance Alzheimer’s and other dementia awareness efforts and deliver disease education to those who are disproportionately impacted and underserved. As a result of his leadership, the Association has broadened its reach to previously underserved communities, with over 30 national partners such as the Thurgood Marshall College Fund, National Indian Council on Aging, Omega Psi Phi Fraternity, Inc., National Black Nurses Association, National Association of Hispanic Nurses, African Methodist Episcopal Church, and the Buddhist Tzu Chi Foundation. In addition, under Dr. Hill’s guidance, the Association has pursued innovative partnerships, such as collaboration with GDavis Productions and Films to develop “Unforgettable,” a community-focused stage play that focuses on a family navigating Alzheimer’s disease and caregiving.

Nathaniel A. Chin, MD

Nathaniel Chin, MD, is a dedicated medical professional with a strong academic foundation and a passion for advancing Alzheimer’s disease research and care. He holds an MD from the University of Wisconsin School of Medicine and Public Health. Following his internal medicine residency at the University of California, San Diego, Dr. Chin returned to the University of Wisconsin and pursued fellowships in geriatrics and dementia.

Dr. Chin currently plays vital roles as the Medical Director and Clinical Core Co-Leader for the Wisconsin Alzheimer’s Disease Research Center (ADRC) as well as the Medical Director for the Wisconsin Registry for Alzheimer’s Prevention (WRAP). His career trajectory was profoundly influenced by his father’s Alzheimer’s diagnosis during his residency, prompting him to become a geriatrician and scientist focused on Alzheimer’s disease and related dementias. He is the Associate Director of the UW Geriatric Memory Program and sees patients two days each week at the UW Health Memory Clinic.

Dr. Chin’s impact extends beyond clinical practice – he also hosts the informative podcast “Dementia Matters” for the Wisconsin ADRC. Through this platform, he engages bi-weekly with Alzheimer’s experts to discuss research breakthroughs and caregiver strategies, enriching awareness and understanding.

At the heart of his pursuits lies a commitment to research, exploring modifiable risk factors and biomarker disclosure within his work at the Wisconsin ADRC and WRAP. Dr. Chin collaborates closely with fellow researchers and oversees the clinical well-being of research participants, embodying his holistic approach to advancing dementia research and patient care.

Statement of Need

Patients from aging populations face a number of challenges when accessing health care, including overt negative bias toward older adults, complex and time-consuming care, and the lack of thorough integration into e-health systems. Patients who find themselves at the intersection of ageism and racism contend with implications like delays in care, exclusion from clinical trials, and increased morbidity and mortality from preventable diseases. Caregivers for elders work in a stressful environment, and the aging population is only growing, so it is essential to provide education and share best practices toward establishing age-friendly health systems.

During this concise, on-demand educational discussion, expert panelists will advocate for aging populations and their caregivers, revealing the structural pressures that enable ageism and racism to persist, and highlighting strategies to address health-related social needs for this underserved population.

Learning Objectives

  • Identify the unique health-related social needs of patients from aging populations and their impact on clinical care practices.
  • Incorporate strategies into clinical practice to address the health-related social needs of patients from aging populations and improve outcomes.
