Livestream: IBD, IL-23, and Inflammation, Oh My! Following the Yellow Brick Road in Using IL-23 Targeted Therapies in Managing IBD

In this CME Outfitters livestream symposium, expert faculty will instruct learners on the role of pro-inflammatory cytokines in the pathogenesis of IBD. Racial/Ethnic Disparities in Dermatology Care: Real-World Strategies to Address Inequities in Treatment and Outcomes

Despite advances in the treatment and management of dermatologic conditions such as psoriasis, significant disparities in both treatment and outcomes persist for patients from racial/ethnic underserved groups. Patients of color are not only more likely to present with more severe disease, but they are also more likely to receive either no treatment, or only topical treatments, despite wide availability of effective biologic medications. In fact, Black patients with moderate to severe psoriasis are 70% less likely to receive biologic treatment than their White counterparts. It is imperative that each member of the multidisciplinary care team works together to address these disparities in order to improve equity in treatment and outcomes for all patients.

In this CMEO BriefCase, expert faculty will utilize case-based learning to illustrate these pervasive disparities through the stories of two patients, Miguel and LaToya, while offering strategies to integrate therapies that optimize long-term outcomes and address racial/ethnic inequities in the diagnosis and treatment of patients with psoriasis.

Optimizing Care for Diverse Patient Populations with Migraine: An Exploration of Migraine Management with a Focus on CGRP Receptor Antagonists

Migraine is the most common neurological disorder, and the second most common cause of disability in the world. Despite the enormous cost it exacts on patients, the healthcare system, and the greater economy, migraine remains largely undertreated or un-treated. Racial disparities in treatment and outcomes also persist. Even though migraine prevalence is very similar among White, Black, and Latino/Hispanic groups, Black and Latino/Hispanic patients are less likely to even receive a migraine diagnosis than their White peers. Even after diagnosis, patients face unmet treatment needs. Common medications used to treat migraine attacks are poorly tolerated, contraindicated, or simply ineffective in some patients. Newer disease-process-specific therapies may offer hope to patients for whom traditional therapies are ineffective, poorly tolerated, or contraindicated.

During this educational presentation, migraine experts will share real-world examples of screening, diagnosis, and implementation of appropriate pharmacotherapy factoring in social determinants of health and patient access to care.

When implementing anticoagulants to prevent thrombotic events (eg, stroke, pulmonary embolism, deep vein thrombosis) in non-valvular atrial fibrillation (NVAF) and/or venous thromboembolism (VTE), it is critical to balance efficacy with bleeding risk. Direct Oral Anticoagulants (DOACs) have come of age and are proving to be an efficacious and convenient means to provide anticoagulation for many patients with NVAF and/or VTE. However, striking the right balance between anticoagulation and bleeding risk is paramount, especially for patients with chronic kidney disease. This resource, adapted from the KDIGO (Kidney Disease Improving Global Outcomes) guidelines, provides a quick reference to warfarin and DOAC dosing, including adjustments for creatinine clearance (CrCl) in patients with various levels of renal insufficiency.

Racial/Ethnic Disparities in Mental Health Care: Real-World Strategies to Address Inequities in Treatment and Outcomes

Though the overall prevalence of major depressive disorder (MDD) is estimated to be largely equivalent across racial/ethnic groups, there is a significant disparity in the rate of diagnosis of MDD for many racial/ethnic underserved communities, including Black and Hispanic groups. In addition, amongst those from these communities who do receive a diagnosis, treatment offerings are often limited, with antidepressant therapy as well as psychotherapy offered at lower rates as compared to their non-Hispanic White counterparts. Social determinants / drivers of health play an additional role in these disparate outcomes, as negative drivers of health have been associated with progression to treatment resistant depression (TRD). It is imperative that each member of the multidisciplinary care team works together to address these disparities in order to improve equity in treatment and outcomes for all patients.

Actionable Opportunities for Molecularly Classified Endometrial Cancers

Multiple clinical practice guidelines for the treatment of endometrial cancer (EC) recommend consideration of molecular testing, particularly for high-grade tumors. Yet despite its prognostic value, molecular classification is not always a part of standard risk assessment in EC. Clinical uptake has been highly variable, particularly in limited-resource areas, even though recent data shows molecular classification has value in therapeutic decision-making and is prognostic for treatment response in EC. Lack of clinical uptake has resulted in overtreatment of some EC patients and undertreatment of others.

Social Determinants of Health (SDoH) and Interstitial Lung Disease (ILD)

One size does not fit all in ILD. To achieve the best quality of care and patient outcomes, clinicians need to assess key factors that impact each individual patient’s ability to access and adhere to recommended lifestyle and therapeutic interventions. Such factors — collectively known as “social determinants of health” include health care access and quality, neighborhood and built environment, social and community context, economic stability, and education access and quality. This clinician tool includes definitions and a questionnaire to facilitate patient interviewing about these important considerations.

SDM, the SHARE Approach, and the Role of the ILD MDT

Shared decision making (SDM) is a key component of patient-centered health care. The SHARE Approach is a five-step process for SDM that provides a framework for exploring the benefits and risks of various therapies and interventions. This top-line reference includes the SHARE acronym and related “conversation starters” to facilitate SDM with patients. On the flip side of the document is a visual reminder of the team approach to ILD care, indicating roles and responsibilities of the various disciplines, that when working together, can lead to improved quality of care and patient outcomes.
